Tryout Information and FinalForms
* All physicals that are dated April 15, 2025, or after are good for the 2025-2026 School Year. (Example: if you get a physical for off-season football or cheerleading tryouts prior to April 15th, 2025 then you will have to plan to get another physical before the first official practice after summer workouts. The termination date for that physical falls on the first official practice.
* All students/athletes who are wanting to tryout, must complete a FinalForms Account on FinalForms. The parent must create a parent login and then a student account completing the physical information, forms, documents and upload the correct media (physicians physical form, insurance cards, and/or Doctors notes with specific cases). All of this information must be completed before you are able to tryout. An incomplete or partially completed form online will not be acceptable to tryout.
* The 2025-2026 Athletic Participation Packet has been updated to include the New Updated Physical Forms. You should get this packet from a coach or the packet can be found in the "Forms" section to the right in the Main Athletics page.
