Please use the New Updated Physical Form from TSSAA when you get your physicals for the 2025-2026 school year.  The new forms are in the "Forms" section on the Main Athletics page.  
 
You can still print off the Participation Packet but replace the physical forms with the new updated forms.  Once all forms have been updated, the packet will be updated.
