Registration Information

6th grade registration information for students from

John Colemon, Smyrna Elementary, Smyrna Primary School, Stewartsboro and Thurman Francis

 

Attention 6th graders coming from our feeder schools. 

 You will not need to pre-register

 

New Student Registration  SUMMER HOURS

Due to construction on our building, registration will be held in the main office at the back of the school (directly behind the football stadium).

 

 

May 31st through June 8th - 8 a.m. until 10 a.m. each day

July 24th through August 4th - 8 a.m. until 10 a.m. each day

 

Items needed for registration. 

 

 

  • Proof of address-electric, water or lease agreement
  • Shot records with proof of physical on TN form

 

(7th graders require additional shots this year - please talk to your healthcare provider)

 

  • Birth Certificate
  • Legal Documents regarding child custody

Welcome to Smyrna Middle School. We look forward to meeting your child in August. I wanted to share a few things that may be helpful for registration. Any 6th grader coming from one of the schools listed above will not need to pre-register. Your child will need to attend the 2 hour day on Monday, August 7th and will be given a registration packet for you to complete. Please return the completed packet to school the following day. For your proof of address, please attach an electric bill, water bill, or lease agreement.

Per Rutherford County Board of Education Policy these are the only types of bills accepted for proof of residency.

7th Grade Immunizations Requirements
Parents:  THIS IS A REMINDER THAT STUDENTS WILL NOT BE ABLE TO START CLASSES IN THE 7TH GRADE WITHOUT THE REQUIRED IMMUNIZATIONS DOCUMENTED ON THE CORRECT FORM, PROPERLY COMPLETED BY A HEALTH CARE PROFESSIONAL.  The vaccine requirements for 7th grade are:  1) Tetanus-diptheria-pertussis booster (Tdap) AND 2) Verification of immunity to Varicella.  The student must have had either 2 doses of the vaccine or a history of having had chicken pox.
